Community Health

Community Health Representatives (CHR’s) are here to assist and guide Tribal Members in finding health services and programs that are available for them. They can help Tribal Members to navigate through the healthcare system, offering support, education and guidance. CHR’s are able to take blood pressure, other basic vitals in the home, and assist with organizing medications. CPR and First Aid classes are available as well.Community Health Representatives have completed training in home healthcare. They are able to provide respite services, bathing, and light housekeeping. They will work with your Healthcare provider and Community Health Nurse to assist with care with the goal of remaining in your home. Please feel free to discuss with your Provider and or contact one of our CHR’s for more information pertaining to these services.
